If you are involved in a New York car accident, the first thing you should do is call the police to file an accident report. If your accident occurred in Queens, you need to contact one of the Queens precincts of the New York Police Department. Contact the precinct office closest to your accident scene:

Accident reports are available for 30 days following the date of the accident. You will need to complete a REQUEST FOR COPY OF ACCIDENT RECORD [PD301-165] form and pay $10 in the form of check or money order, payable to “City of New York, Police Department.” Take your completed form and payment to the precinct office.

After 30 days, your report can only be accessed from the New York State Department of Motor Vehicles. You will need to submit a NYS DMV form MV-198C form and pay a $20 fee.

If your accident occurred outside of Queens, you can determine the appropriate agency to contact by calling police stations or precinct offices nearest to the accident scene. If your accident occurred outside of a city or town, begin by calling the New York State Police or the county sheriff’s office. You will need to provide the date and location of the accident. You will also need to provide the names of the drivers and passengers involved in the crash.
